This is not an application and it is not sent anywhere. It is a worksheet: bring it to an accredited centre and the registerer will type from it. Filling it in at home turns a long queue into a few minutes at the desk.

Serving members — bring your PSID number. With it you pay no fee at all. Without it you are charged ₦1,050. A new member pays ₦2,050. Pay only by transfer to the NHQ approved consultant account — Societrics Ltd, 1312721742, Zenith Bank. Never hand cash to anybody.

Your photograph is taken at the centre. You must attend in person — nobody can be registered in their absence.
